The 2026 guide to choosing an SHA-compliant HIMS in Kenya

When the Social Health Authority (SHA) replaced NHIF in October 2024, it did more than change a logo. The claims architecture moved from paper batches and Excel schedules to real-time API submissions with structured clinical bundles. Hospitals still running pre-SHA HIMS are seeing rejection rates north of 20% — and payments delayed by 60+ days.

1. Does the HIMS submit claims via the SHA API?

Not a portal upload. Not a downloaded template. A direct, authenticated API integration with retry, idempotency and delivery receipts. Ask for a live demo of a claim moving from encounter closure to acknowledged submission in under 30 seconds.

2. Are clinical encounters modelled as FHIR resources?

SHA's structured bundles are FHIR R4. If your HIMS stores encounters as free text and only serializes them at submission time, expect data-quality rejections. FHIR-native systems catch missing coded diagnoses, missing observations and missing dispensing lines at the point of documentation.

3. Pre-authorization: minutes, not days

SHA services above defined thresholds need pre-auth. A compliant HIMS asks for pre-auth at order entry, submits automatically and updates the claim state without a claims officer opening a portal.

4. Denial management is a first-class feature

Every claim will occasionally be denied. What matters is how fast you resubmit. Look for: denial reason capture, one-click correction, resubmission audit trail and denial-rate dashboards by clinician, service and payer.
